MAPTA reserves the right to refuse/cancel an event registration. If MAPTA refuses a registration, registrants will be offered a full refund.

  • A $25.00 administration fee will be withheld when cancellation occurs up to ten days prior to the start of the event; 50% refund if cancelled 2-10 days prior; 25% refund if cancelled 0-1 day prior.

Course Description: 

Perioperative Pain Neuroscience Education
The perioperative period is filled with stress, anxiety and fear. All of these psychosocial factors have beenassociated with poor outcomes. From a neuroscience perspective, the perioperative period is associatedwith a hypervigilant nervous system. This heightened state of the nervous system and various psychosocialissues have been the target of various perioperative interventions for more than a quarter of a century,including preoperative education, pre-emptive analgesia, etc. The culmination of this work is the routinepreoperative education classes given to patients prior to surgery. From an orthopedic perspective,preoperative education is most prevalent in total knee and total hip arthroplasties. To date, however, threesystematic reviews have shown no postoperative benefit to these programs in regards to postoperativepain, range of motion, length of hospital stay and function. Emerging pain neuroscience research hasshown that these educational models fail partly due to a heavy focus on procedural and anatomicaleducation, with little to no attention being given to pain-specific education. A newly designed preoperativepain neuroscience education program has shown that teaching patients more about pain prior to surgeryleads various positive outcomes including significant decrease is healthcare utilization after surgery, patientsatisfaction and more. This class aims to introduce attendees to the development of the preoperativeneuroscience education session, the content, delivery methods and clinical application of such a programfor lumbar surgery and total knee arthroplasty.

A Pain Science Approach to Whiplash
Whiplash associated disorders (WAD) are clinically challenging and epidemiological studies currently show thatapproximately 30-40% of patients involved in motor vehicle collisions move into chronicity. Several authors haveimplicated various factors associated with the high prevalence of chronicity, including biomechanical reasons, copingbehaviors, bio-psycho-social yellow flags, environment and poor management in the acute phase. Modern painscience, however, has shown a significant hypervigilance of the nervous system within 7-10 days post motor vehiclecollision, which has been correlated to persistent pain. This 1-day, 8-hour lab class will delve into the latest painscience view of WAD including central sensitization, peripheral sensitization, and receptor field changes, functionaland structural changes in the brain – all which are associated with persistent pain. WAD is more than “just” tissueinjury and assessment and treatment of the hypervigilant nervous system is needed in the search for recovery inWAD. This class will include the latest clinical screening and tests including sensory discrimination, laterality, two-point discrimination, localization, pressure pain thresholds and more. Treatments will include pain neuroscienceeducation, graded motor imagery, sensory discrimination, neurodynamics, manual therapy, motor control,sensorimotor retraining and more.

Upon completion of this educational session the participants will be able to:

1. Understand why a new bio-psycho-social approach was needed to address pain in orthopedicsurgery

2. Understand the development and validation process of the preoperative neuroscience educationprogram for orthopedic surgery

3. Be able to understand the content and delivery methods for the preoperative neuroscienceeducational program

4. Recognize why the preoperative neuroscience educational program produced superior results tothe traditional biomedical model being utilized 

5. Apply the information from the educational session into clinical practice

Whiplash Course Objectives:

Upon completion of this educational session the participants will be able to:

1. Develop a pain science understanding of how WAD’s are “more than just” tissue injuries

2. Recognize the various neurobiological, physiological and immune mediated processes associated with ahypervigilant nervous system in patients with a WAD

3. Develop a clinical understanding of the common signs and symptoms of a sensitized nervous system in apatient with a WAD

4. Recognize how a more comprehensive neuroscience treatment may be warranted in patients with a WAD

5. Safely and effectively use the tests and treatments used in the class6. Implement test and treatment principles from the lecture into clinical practice

About the Speaker

Brett Neilson, PT, DPT, OCS, TPS, FAAOMPT

Brett earned his Doctorate of Physical Therapy at the University of Puget Sound, in Tacoma, Washington.  He is Board Certified in orthopedic physical therapy and is a Fellow in the American Academy of Orthopaedic Manual Physical Therapy. He completed an Orthopaedic Physical Therapy Residency and Orthopaedic Manual Physical Therapy Fellowship with Evidence in Motion Institute of Health Professions and his Therapeutic Pain Specialist certification with the International Spine and Pain Institute. Brett currently serves on the Board of Directors for the Physical Therapy Association of Washington and as a Nominating Committee member for the Pain Management SIG of the APTA. Brett has served in many roles such as clinic director in private practice, as a contractor for the US Air Force and as Trial Coordinator, coordinating the day-to-day activity on two large multi-center randomized controlled trials in knee osteoarthritis and low back strengthening for Army combat medics. Currently he practices in Kent, WA with Outpatient Physical Therapy, a private practice group, where he mentors EIM residents and treats patients with chronic pain and movement dysfunctions. He is also on staff with EIM and serves as their Admissions Director and adjunct faculty. Brett also serves as Adjunct Faculty for South College Doctorate of Physical Therapy program in Knoxville, TN and ISPI.
